I changed the name of a major character between first draft and revisions (and am still, now that I'm going through revisions, fixing some places where search + replace didn't change the name correctly). See, one POV character had seen the character's name written down and one hadn't, only heard it, and it was a slightly non-standard spelling. It bugs me when people can automatically tell how a name is spelled just by hearing it in books (or at least when the narration automatically knows, though the character doesn't) so I had spelled it one way in one character's POV and the other way in the other character's, but I decided that would be too confusing for readers. So I changed it to another name entirely.
